Achilles tendonitis is inflammation or degeneration of the Achilles tendon — the large tendon connecting your calf muscles to your heel bone. It causes pain at the back of the heel and lower calf, typically worsening with activity. Common in runners and active individuals.
Acute Achilles tendonitis often improves in 4-6 weeks. Chronic cases (tendinopathy) where the tendon has thickened may take 12-16 weeks of specific loading exercises. The key is progressive tendon loading — complete rest actually delays healing.
Complete rest is counterproductive for Achilles tendonitis. The tendon needs controlled, progressive loading to heal. We prescribe specific eccentric exercises (heel drops) that have strong evidence for Achilles recovery, starting gently and building gradually.
You should reduce running volume and avoid hill running and sprinting initially. Many patients can continue gentle running if pain stays below 3/10 during and after. We create a modified running plan that allows tendon healing while maintaining fitness.
Sudden increases in running distance or intensity, tight calf muscles, poor footwear, and running on hills are the main causes. Age-related tendon changes (after 30) also increase vulnerability. We address all contributing factors in treatment.
